Procedure for Incident Reporting

The procedures for Coaches and Team Managers reporting an injury, an incident involving violence or abuse or any breach of a Club Code of Conduct are detailed below.

Reporting an Incident causing Injury or Significant Injury
1. Identify the extent of the injury (fracture / strain / sprain requiring medical attention) and assess the need for further treatment or time off work
2. Treat injury, involving St John’s / Ambulance (000) where necessary  
3. Ensure referee records injury on the match sheet at end of game (ie. before managers from each team sign the match sheet)  
4. Complete AJFC Incident Form and deliver with match sheet (if possible).  If playing an away match, complete the form as soon as possible after the match and email a copy of the completed form to the Club at secretary@ajfc.net.au.
5. Club Secretary reports the incident to Association Secretary  
6. Injured player to complete an online insurance claim and submit within 60 days of date of injury at link below:

Reporting an Incident involving Violence or a breach of Code(s) of Conduct 
1. Ensure safety of children – remove from field if necessary. A referee should deal promptly with these issues on field. Off field, the Ground Official (whether home or away) should deal with incident. If needed, seek help from a Club Committee member at our home ground.
2. Ensure referee notes incident on match sheet at end of game (ie. before managers from each team sign the match sheet)
3. Complete an AJFC Incident Form and deliver with match sheet. If playing an away match, complete the form as soon as possible after the match. Email a copy of the completed form to the Club at secretary@ajfc.net.au. Do this regardless of whether the incident occurred at home or at an away venue. Include player card and/or shirt numbers.  
4. Club Secretary or President to report to Association with appropriate follow-up with Judiciary if required  
5. Club Secretary or President to feedback to Ground Official re outcomes
